BY THIS CHANGE OF NAME DEED made this 25th day of October One Thousand Nine Hundred and Ninety Nine, I the undersigned ANNIE RACHEL JENNIFER BRION (Date of Birth: 13/02/1965) of FELTHAM, MIDDLESEX DO HEREBY ABSOLUTELTY RENOUNCE AND ABANDON the use of my former name of ANTHONY ROBERT JAMES BRION and assume adopt and determine to take and use from this date hereof the name of ANNIE RACHEL JENNIFER BRION in substitution for my former name of ANTHONY ROBERT JAMES BRION AND in pursuance of such change of my former name of ANTHONY ROBERT JAMES BRION as aforesaid I shall at all time hereafter in all records deeds documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever subscribe the said name of ANNIE RACHEL JENNIFER BRION as my name in substitutions for my former name of ANTHONY ROBERT JAMES BRION so relinquished as aforesaid to intent that I may hereafter be called known or distinguished not by the former name of ANTHONY ROBERT JAMES BRION but by the name of ANNIE RACHEL JENNIFER BRION only AND I HEREBY AUTHORISE and require all persons at all times to designate describe and address me by the adopted name of ANNIE RACHEL JENNIFER BRION
